If you using Venue EMT, the following code can be used to build subscribe forms.
Remember to substitute your sites id where you see YOUR_ID.
Note: The only required text field is email_address.
Optional Settings:
If you do not want to require that the email address be entered twice (after the form is submitted) then add this hidden input to the form.
